Services

Angel's Family is licensed under the Colorado Department of Public Health and Environment to provide the following services in Jefferson, Denver, and Arapahoe counties:
Residential Services provide individuals with 24-hour access to supervision and support. Individuals may choose to live and receive services in the family home, or may elect to live and receive services outside of the family home. The level of supervision and support provided to the individual in each setting is based on the assessed individual need.
Individual Services and Supports (IRSS) uses a variety of living arrangements designed to meet each individual’s needs. The individual may live in a home owned or leased by an agency, their own home or a host home. IRSS are generally provided to no more than two persons receiving services per setting, although three person settings are allowed under certain circumstances. Personal Care Alternative programs include host homes, clustered or staffed apartments. No more than three individuals may live in any one apartment. Twenty-four hour accessible supervision and support are available. Options for living within the family home include comprehensive services in the Family Home and Family Caregiver.

Family Caregiver allows for residential services to be provided in the family home by a family member under the Family Caregiver Act. To be a paid provider, family members must become an employee of a program-approved service agency.
Supported Living Services (adults) are used to supplement already available supports for adults who either can live independently with limited supports or, if they need extensive support, are principally supported from other sources, such as the family.
 
Supported Living Services (SLS) provide a variety of services, such as personal care (like eating, bathing and dressing), homemaking needs, employment or other day-type services, such as accessing his or her community, help with decision making, assistive technology, home modification, professional therapies, transportation and 24-hour emergency assistance. SLS are not intended to meet all needs. Service needs and the level of support are prioritized within the overall individualized plan (IP).
 
The person receiving services is responsible for his or her living arrangements, which can include living with family or in their own place.  Up to three persons receiving services can live together. The person receiving services cannot require 24-hour supervision paid for only by SLS.
Non-Medical transportation for any clients under (HCBS-DD and HCBS-SLS).
Children's Extensive Support (HCBS-CES) is a program that provides Medicaid-funded services and support to children with developmental disabilities or delays who have the most intensive behavioral and/or medical needs, and are at high risk of out-of-home placement. Eligible children demonstrate a behavior or have a medical condition that requires direct human intervention, more intense than a verbal reminder, re-direction or brief observation of medical status, at least once every two hours during the day and on a weekly average of once every three hours during the night. The behavior or medical condition must be considered beyond what is typically age appropriate and be due to one or more of the following conditions:
  1. A significant pattern of self-endangering behavior(s) or medical condition that, without intervention, will result in a life-threatening condition or situation.
  2. A significant pattern of serious aggressive behaviors toward self, others or property.
  3. Constant vocalizations (on average of 15 minutes of each waking hour), such as screaming, crying or laughing.
Respite-Individual (per 15 min)

The individual will receive respite in a one-on-one situation. There will be no other individual in the setting also receiving respite services. Individual respite will occur for 10 hours or less in a 24-hour period. Respite will be provided in the individual’s own home or in the home of a trained respite provider. Angel's Family, staff or contractors will not provide hands-on care during paid respite time, such as toileting, bathing, dressing, continence care, medication assistance, transferring, mobility or personal laundry.
Respite-Individual (per day)    

The individual will receive respite in a one-on-one situation. There will be no other individuals in the setting also receiving respite services. Individual respite will occur for cumulatively more than 10 hours in a 24-hour period. Respite will be provided in the individual’s own home or in the home of a trained respite provider. Angel's Family staff or contractors will not provide hands-on care during paid respite time such as toileting, bathing, dressing, continence care, medication assistance, transferring, mobility or personal laundry.

Home Healthcare Services include skilled nursing, at-home physical therapy, pain management, wound care, prescription management, meal preperation, companionship and friendship, housekeeping, personal care, and more (all these are decided upon initial evaluation by our registered nurse).
Basic Homemaker are services that consist of the performance of basic household tasks within the participants’ primary residence (i.e. cleaning, laundry or household care), including maintenance related to the participant’s disability and provided by a qualified homemaker, when the parent or primary caretaker is unable to manage the home and care for the participant in the home. This assistance must be due to the participant’s disability that results in additional household tasks and increases the parent or caregiver’s ability to provide care needed by the participant. This assistance may take the form of hands-on assistance (actually performing the task for the participant) or cuing to prompt the participant to perform the task.
Enhanced Homemaker is when Angel's Family will ensure that services will be provided by a qualified homemaker that consist of the same household tasks as described under basic homemaker services, with the addition of either habilitation or extraordinary cleaning. Habilitation will include direct training and instruction to the participant, which is more than basic cuing to prompt the participant to perform a task. Habilitation will include a training program with specific objectives and anticipated outcomes that may be some amount of incidental basic homemaker services that is provided in combination with enhanced homemaker services. However, the primary intent must be to provide habilitative services to increase independence of the participant. Habilitation will include some hands-on assistance (actually performing the task for the participant) or cuing to prompt the participant to perform a task, only when such support is incidental to the habilitative services being provided and the primary duties must be to provide habilitative services to increase independence of the participant. Enhanced homemaker services also include the need for extraordinary cleaning as a result of the participant’s behavior. Angel's Family will assure that any individual providing services will be 18 years old and will have training and experience commensurate with service or supports being provided.
Mentorship will assure that services Angel's Family provide to participants is to promote self-advocacy through methods such as instructing, providing experiences, modeling and advising. This service includes assistance in interviewing potential providers, understanding complicated health and safety issues and assistance with participation on private and public boards, advisory groups and commissions. This service may also include training in child and infant care for parents who themselves have a developmental disability. This service does not duplicate case management or waiver services such as day habilitation. Angel's Family will assure that any individual providing this service is at least 18 years old and has training or experience commensurate with the service or support being provided.
Community Connector services support the abilities and skills necessary to enable the individual to access typical activities and functions of community life such as those chosen by the general population. Community Connector will provide a wide variety of opportunities to facilitate and build relationships and natural supports in the community while utilizing the community as a learning environment to provide services and supports as identified in the participant’s service plan. These activities are conducted in a variety of settings in which participants interact with non-disable individuals (other than those that are providing services to the participant). These types of services may include socialization, adaptive settings, resources necessary for participation in activities and supplies related to the skills acquisition, retention, or improvement. Community Connector is provide on one- on-one basis as a learning environment to provide instruction when identifies in the Service plan. Angels Family will assure that any individual providing this service is at least 18 years old and has training or experience commensurate with the service or support being provided.
All services above will be provided at the person’s receiving services homes. None of these services will be provided in any of Angel's Family facilities.
All services are provided to children and adults with intellectual and developmental disabilities.
The Children's Habilitation Residential Program Waiver (CHRP) provides residential services for children and youth in foster care who have a developmental disability and very high needs. Their needs for support put them at risk for institutional care. Waiver services help children and youth learn and maintain skills needed to live in their communities.
